2010-2011 Updated Medical Policies & Forms

Please click here to download and view our updated medical policies and forms for the 2010-2011 school year.  These forms are due by July 30.

We are now requiring students to complete a comprehensive medical form which includes clearances for participation in physical education and athletic activities as well as an immunization history.  As of 2008, all students must also enclose a copy of their most recent dental screening with their physical exam form.

Important Message from School Nurse's Office:

May 2010

While we were spared any significant outbreak of illness during the winter, it seems that we are now experiencing an increase in the number of student and staff illnesses. We have noticed an increased number of illnesses with flu-like symptoms such as vomiting and diarrhea, and a few students and faculty with strep throat and fifth disease. It is important to note that with fifth disease, the initial illness is very mild (sore throat, runny nose and low grade fever) with a rash developing after a few days. By the time the rash appears the person is no longer contagious.

To prevent further outbreak and minimize student absences, we would like to review the Student Illness Policy:

  • Any child complaining of illness during the school day will be evaluated by the school nurse who will then inform a parent/guardian.
  • Any child who has a fever (temperature of 100.3 ºF or over), nausea or vomiting will be sent home.
  • Any child who exhibits signs or symptoms of a communicable disease must be sent home.
  • Any child who is unable to participate in school activities due to illness must be sent home.

Students must be free from fever, nausea, and vomiting for a minimum of 24 hours prior to returning to school. This assumes 24 hours without any fever reduction medicines. If your child returns to school the day after being sent home with a fever, they will not be permitted to attend. While we understand that a child’s illness can be extremely inconvenient since many families do not have a stay-at-home parent, every parent must understand that one sick child can cause many other student and faculty illnesses. It is very important that every family have a plan in place for child care in case your child cannot attend school.

Our primary concern is always for the health of our students and we feel that as a community we can decrease the risk to all of our students and staff by following the illness policies. If you have any questions or concerns, feel free to contact our school nurse, Wendy Freeburn, at 718-833-9090 or via e-mail at wfreeburn@bayridgeprep.org.
